A useful referral begins with the relevant clinical and social context.

CHNSS considers referrals within its current scope and Queensland service area. The referral pathway helps clarify whether CHNSS is appropriate and what next step may be most useful.

Appropriate referrals

People and families who may benefit from independent nursing guidance, community nursing support, healthcare navigation or coordinated next steps.

Who may refer

GPs, specialists, hospital clinicians, nurses, social workers, allied health professionals, care coordinators, community organisations, providers, families and individuals.

Helpful information

Client contact details, reason for referral, relevant context, current supports and the client’s preferred contact approach. Share only information relevant to the referral.

Choice and consent

Discuss the referral with the client where appropriate and support their choice about whether CHNSS contacts them. CHNSS will confirm consent and suitability directly.

Nursing support within the existing CHNSS scope.

CHNSS helps people and families understand options and coordinate appropriate next steps. It does not replace the treating team, hospital, GP, specialist, provider or funded support system.

CHNSS communicates with the referrer only where the client has authorised this and it is appropriate.

Service access & boundaries

Queensland-based, with appropriate pathways discussed individually.

CHNSS currently serves people and organisations across Queensland, Australia. Service availability, funding or payment pathways and any remote consultation options are discussed through the relevant existing CHNSS and secure practice pathways.

CHNSS is not an emergency service. In an emergency, call 000 or attend the nearest Emergency Department.
